Integrating Pharmacist-Led HIV PEP Services Through Collaborative Care
Daniya Mathew, PharmD, AAHIVP

 Bio: 

Daniya S. Mathew graduated from St. John’s University College of Pharmacy and Health Sciences in May 2022 and subsequently completed her PGY1 pharmacy residency at North Shore University Hospital and PGY2 ambulatory care pharmacy residency at Northwell Health. Dr. Mathew is a Clinical Pharmacy Specialist at Northwell Health/Vivo Health Pharmacy in the Ambulatory Pharmacy Support Services department. Dr. Mathew also serves as Course Director for Vivo Health Pharmacy’s Continuing Education series. Dr. Mathew is a proud delegate and new practitioner committee member for the Long Island Society of Health System Pharmacists. 


Medical Aid in Dying: A Legal, Ethical, and Clinical Primer
Michaela L. Myerson, PharmD, BCPS


Bio:

Dr. Michaela L. Myerson, Pharm.D., BCPS is a clinical pharmacist and a specialist in investigational drugs and research who serves as the Investigational Pharmacy Manager at NYU Langone Health. In her role, she supports a team of research pharmacists and technicians who contribute to over 600 clinical trials, about 80% of which are oncology trials. These clinical trials are often a last resort for people living with cancer, rare diseases, and difficult to treat conditions. Dr. Myerson also sits on the NYU Langone Health Institutional Review Board, which is responsible for ensuring human subjects protections are maintained in ongoing clinical research studies.

Dr. Myerson graduated with her Pharm.D. from the University of New England. She completed her PGY-1 residency at Mount Sinai Beth Israel, and her PGY-2 residency in Investigational Drugs and Research at Brigham and Women’s Hospital.

Outside of work, Dr. Myerson has spent over 15 years advocating on Capitol Hill for people with orphan and rare diseases and is currently the Co-Chair of the NYSCHP Advocacy Committee. Dr. Myerson has been awarded by the Cystic Fibrosis Foundation and the U.S. Public Health Service for her advocacy and public health involvement.

Benjamin Pastore, PharmD

Bio:

Benjamin Pastore is a current PGY-2 adult oncology pharmacy resident at Memorial Sloan Kettering Cancer Center in New York, NY. He earned his PharmD at the University of New England in Portland, ME, then completed his PGY-1 pharmacy residency at Brigham and Women’s Hospital in Boston, MA. He is an active member of HOPA and Alpha Zeta Omega. His favorite part of his job is the impact he sees pharmacists make on patients’ lives, providing their expertise with patient education and pharmacotherapy management from behind the scenes to the bedside.

Travis B. Dick, PharmD, MBA, FASHP, BCPS, CPEL

Bio:

Travis Dick earned a Bachelor’s degree in Biochemistry from the University of New Mexico followed by a Doctor of Pharmacy from the University of North Carolina-Chapel Hill. He then completed both PGY1 and PGY2 pharmacy residencies at Duke University Hospital where he specialized in Solid Organ Transplantation. After residency Travis relocated to Utah where he was a Transplant Clinical Pharmacy Specialist & Clinical Coordinator for the transplant team.  He earned a Master’s degree in Business Administration at the University of Utah, is recognized as a Certified Pharmacy Executive Leader, is a Board Certified Pharmacotherapy Specialist, and ASHP Fellow that has presented at state, national, and international meetings.

Travis is currently the Director of Clinical Pharmacy Practice, Research, & Education and is the PGY1 Pharmacy Residency Director at the University of Rochester.  He is the Immediate Past President of NYSCHP, serves as Delegate for ASHP, is a member of the ASHP Council on Education & Workforce Development, International Accreditation Commission, and Practice Advancement Initiative Advisory Panel. He is a Past-President of the New York State Chapter of ACCP, past Chair for the ASHP Frontline Leaders Section Advisory Group, past Chair of the ASHP SPPL Educational Steering Committee, and past board member for the Utah State Society of Health-System Pharmacists.

Nicole Webb Donovan, PharmD, BCIDP

Bio:

Dr. Nicole Webb Donovan is the Clinical Coordinator and PGY 1 Pharmacy Residency Program Director at Niagara Falls Memorial Medical Center. As a board certified infectious diseases pharmacist, she collaborates with teams across the organization to advance antimicrobial stewardship, enhance medication safety, and support the adoption of evidence based practices.

Dr. Donovan earned her Doctorate of Pharmacy from D’Youville University before completing her PGY 1 Pharmacy Residency at Niagara Falls Memorial and her PGY 2 Infectious Diseases Residency at Buffalo General Medical Center, finishing her advanced training at the start of the COVID 19 pandemic—an experience that reinforced her growing commitment to interdisciplinary collaboration and adaptive, patient centered clinical decision making. She continues this work through residency leadership and experiential training, fostering the development of pharmacy students and residents, collaborating with medical learners, and promoting ongoing development among fellow clinicians.


A Pain in the Acinetobacter - Managing Infections Caused by Multidrug Resistant Gram-Negative Pathogens
Bessma Hassani, PharmD, BCPS, BCIDP

Bio:

Dr. Bessma Hassani serves as the clinical pharmacy specialist in infectious diseases and antimicrobial stewardship at Long Island Jewish Valley Stream. She is a Massachusetts native who received her Doctor of Pharmacy degree from St. John’s University in 2019. Dr. Hassani completed her PGY1 pharmacy residency at South Shore University Hospital, and her PGY2 infectious diseases pharmacy residency at SBH Health System. She is board certified in pharmacotherapy and infectious diseases. Dr. Hassani has been working in her role at Long Island Jewish Valley Stream since 2022. Her practice interests include multidrug resistant gram-negative infections, opportunistic infections, and fungal infections. She also enjoys precepting and mentoring students and residents. In her free time, she enjoys playing drums, walking her dog, and reading novels.

Lisa A. Nelson, PharmD

Bio:

Lisa Nelson serves as the Associate Vice President of IT Applications and Chief Applications Officer for the University of Rochester Medical Center and Affiliates. In this significant role, she acts as Co-CIO, contributing to the leadership of the Information Systems Division. She leads a team of over 650 professionals, overseeing the implementation and support of all Electronic Health Record (EHR) functions, alongside managing more than 500 applications across URMC. Her teams are instrumental in planning, developing, and directing institution-wide efforts for clinical and revenue cycle application implementation, as well as supporting, enhancing, and expanding the Epic EHR, integrated applications, supply chain, and IT training initiatives. Lisa is a strong proponent of an intent-based leadership model and fostering cross-functional collaboration.

Previously, Lisa was the Senior Director of Core Clinical Systems, where her focus was on directly supervising the clinical aspects of the EHR. Prior to that, she held the position of Associate Director, Pharmacy Informatics at the URMC. In this capacity, she was a key member of the URMC Pharmacy leadership team and a Project Director within the Information Systems Division.

Lisa has also made notable contributions through her publications and frequent speaking engagements on topics such as Pharmacy Informatics, Technology, Clinical Pharmacy Initiatives, and Patient Safety. She earned her undergraduate Pharmacy degree from the University at Buffalo, completed her residency at Rhode Island Hospital, and received her Doctor of Pharmacy from the Albany College of Pharmacy. Lisa resides in Rochester, New York.

Barb Rogler, PharmD, MS


Bio:

Dr. Barbara A. Rogler is a Clinical Assistant Professor at the State University of New York at Buffalo, School of Pharmacy and Pharmaceutical Sciences. As the liaison to the New York State Department of Health’s Office of Health Insurance Programs, Dr. Rogler manages and advances the NYS Medicaid Drug Utilization Review Program. She coordinates work plans, leads interdisciplinary teams of analysts and faculty, and authors strategic drug utilization and pharmacoeconomic reports that inform the state's Medicaid formulary and policy decisions.

Prior to her academic appointment, Dr. Rogler held senior clinical leadership roles at various healthcare companies. Throughout her career, she specialized in managed care pharmacy, drug utilization review (DUR), pharmacoeconomic modeling, policy development, formulary management, and the integration of disease management strategies to improve patient outcomes and control healthcare costs.

Dr. Rogler holds a Doctor of Pharmacy from Shenandoah University, a Master of Science from Russell Sage College, and a Bachelor of Science in Pharmacy from Albany College of Pharmacy and Health Sciences.

Lisa S. Stump, BSPharm, MS, FASHP

Bio:

Lisa Stump is an accomplished healthcare executive and digital transformation leader, currently serving as Executive Vice President, Chief Digital Information Officer, and Dean of Information Technology at Mount Sinai Health System. In this role, Lisa leads a team of over 900 IT professionals and data scientists, driving digital innovation to enhance patient care, operational efficiency, and research and educational excellence.  Lisa established the Center of Excellence for Digital Health, AI, and Innovation, and unified business, clinical, and financial reporting across the organization.

Previously, Lisa was Chief Information and Digital Transformation Officer at Yale New Haven Health, where she managed IT operations, led major automation and infrastructure modernization initiatives, and advanced telehealth and data science capabilities. Lisa’s earlier roles include Epic Project Director, and Director of Pharmacy Services, with a track record of successful clinical program development, system implementations and operational excellence.

Lisa holds a Master of Science in Pharmacy Administration from The Ohio State University and a Bachelor of Science in Pharmacy from the University of Connecticut, along with ASHP-accredited residency and multiple professional certificates. Lisa is widely recognized with honors including HIMSS Most Influential Women in HIT, BostonCIO Orbie CIO of the Year, and The Stevie® AI Gamechanger award.

Sum Lam, PharmD, BCPS, BCGP

Bio:

Dr. Sum Lam is a Clinical Professor of Pharmacy Practice at St. John’s University College of Pharmacy and Health Sciences in Queens, New York. In this role, she develops didactic courses in clinical therapeutics, conducts pharmacy research, precepts Advanced Pharmacy Practice Experience (APPE) students, and serves on the departmental Personnel and Budget Committee.  Dr. Lam earned her Doctor of Pharmacy at the University of Connecticut (Honor Scholar), she completed a PGY-1 pharmacy practice residency at Montefiore Medical Center in New York.  Later, she completed a PGY-2 specialized residency in geriatric pharmacy at the Durham VA Medical Center, the University of North Carolina & Duke University Center for the Study of Aging and Human Development.  Dr. Lam has contributed to multiple therapeutic textbooks including Pharmacotherapy: A Pathophysiologic Approach, Pharmacotherapy Principles and Practice & Fundamentals of Geriatric Pharmacotherapy. She is an invited faculty member for the ASHP/ACCP Geriatric Pharmacy Literature Study 2025. She served as the Chair for the Geriatrics Practice & Research Network within American College of Clinical Pharmacy. She is the President-Elect for Long Island Society of Health System Pharmacists (LISHP).


Leadership Pearls: The Power of Being Human: Vulnerability & EQ in Leadership
Doreen Chiu, PharmD, BCPS

Bio:

Dr. Doreen Chiu is the Assistant Director of Pharmacy at Montefiore New Rochelle Hospital, where she provides both clinical and operational leadership in an increasingly complex healthcare environment. With nearly two decades of experience spanning retail and hospital pharmacy, she brings a broad and pragmatic perspective to the evolving challenges facing healthcare systems today.

Beyond her hospital role, Doreen is deeply engaged in professional leadership at both the local and state levels. She serves as Past President and current Social Media Chair of the Westchester County Society of Health-System Pharmacists (WCSHP). At the state level with the New York State Council of Health-System Pharmacists (NYSCHP), she serves as Vice Chair of the Leadership Programming Subcommittee, Co-Chair of the Industry Outreach Committee, and creator and lead of the ASCEND Mentorship Program and the Leadership Book Club.

Doreen has lectured at both the local and state levels on leadership and navigating complexity in healthcare. She believes effective leadership begins with self-awareness and compassion and focuses on helping leaders align people, systems, and purpose toward a shared goal in high-pressure environments.

Sara Radparvar, PharmD, BCPS, BCCCP


Bio:

Sara Radparvar, PharmD, BCPS, BCCCP received her PharmD from Rutgers, and completed her PGY1 Pharmacy and PGY2 Critical Care Pharmacy Residencies at UMass Memorial Medical Center. Before joining Mount Sinai Health System in 2018, she served as an Assistant Professor at Rutgers University, concurrently maintaining a clinical practice site in a mixed medical, surgical, and neurosurgical ICU at RWJ Barnabas. Upon joining Mount Sinai, Sara played an important role in establishing clinical pharmacy services in the SICU. In her current position as the Director of Clinical Services, she is focused on enhancing the integration of pharmacists into the bedside healthcare team and fostering interdisciplinary collaboration on research and quality improvement initiatives. She also serves as the Residency Program Director for the PGY1 Pharmacy Residency Program at The Mount Sinai Hospital. Sara’s professional interests include critical care, anticoagulation management, process improvement, and trainee education.


Leadership Pearls: Advancing Diversion Detection: Intelligent Tools for Identification, Reconciliation, and Workflow Efficiency
Anthony Maddalena, PharmD

Bio:

Dr. Anthony Maddalena is the Associate Director of Pharmacy for Operations at Westchester Medical Center, where he provides operational leadership across pharmacy services at the Valhalla campus. He earned his Doctor of Pharmacy degree in 2011 from the University of Colorado.

Dr. Maddalena brings more than 25 years of progressive pharmacy leadership experience. From 1999 to 2011, he served as Assistant Director of Pharmacy at Burke Rehabilitation Hospital. He subsequently held Operations Manager roles at two additional healthcare organizations from 2011 to 2021 before joining Westchester Medical Center.

Committed to education and professional development, Dr. Maddalena serves as a preceptor for multiple schools of pharmacy, supporting both Introductory and Advanced Pharmacy Practice Experiences. He is also an active preceptor and contributor to the PGY1 pharmacy residency program at Westchester Medical Center.


Expecting Safety: Preventing Medication and Vaccine Errors During Pregnancy
Taylor Clark, PharmD


Bio:

Dr. Taylor M. Clark is a Clinical Assistant Professor at the Binghamton University School of Pharmacy and Pharmaceutical Sciences and an Ambulatory Care Clinical Pharmacist at the United Health Services Heart and Vascular Institute. She earned her Doctor of Pharmacy degree from Binghamton University and completed a PGY-1 Pharmacy Residency with Geisinger Clinic Northeast, where she developed strong expertise in chronic disease management and team-based ambulatory care.

Dr. Clark teaches across the PharmD curriculum in integrated pharmacotherapy, skills laboratories, lifelong learning, and special populations, with a focus on reproductive health and endocrine pharmacotherapy. She is an active preceptor, research mentor, and faculty of the Rural and Underserved Service Track.

Her growing research portfolio spans interprofessional education, chronic disease outcomes, and women’s health. Current projects include a pharmacovigilance analysis of vaccination errors during pregnancy using VAERS data, as well as work evaluating GLP-1 transition strategies and pharmacist impact on chronic disease quality measures. She has published in Annals of Pharmacotherapy and JACCP and has presented at national and state meetings including AACP and NEXUS Summit. Dr. Clark serves as President-Elect of STSHP and Faculty Council Liaison to NYSCHP.


Keynote Session: Beyond the Counter: Amplifying Pharmacy's Voice for Health Equity and Public Health
Christina M. Madison, PharmD, FCCP, AAHIVP

Bio:

Dr. Christina M. Madison, PharmD, FCCP, AAHIVP, is a residency-trained clinical pharmacist, nationally recognized public health advocate, and healthcare media contributor with more than two decades of experience. Known as The Public Health Pharmacist®, she is the Founder and CEO of The Public Health Pharmacist, PLLC, where she focuses on health communication, media strategy, and advancing health equity.

Dr. Madison spent 16 years in academia as faculty at Roseman University of Health Sciences College of Pharmacy, where she trained future generations of healthcare professionals in infectious disease, HIV, reproductive health and justice, and public health. Transitioning from academia, she has become a successful entrepreneur, leading multiple thriving businesses centered on public health consulting, media communications, and professional branding.

She is a two-time TEDx speaker, recognized for her thought leadership and her commitment to amplifying Black and Brown voices through public relations and media strategy. In addition, she serves as a part-time clinical consultant for the UCSF National Clinician Consultation Center, utilizing her decades-long expertise in HIV and communicable disease management to assist providers nationwide. Her expertise has also led to more than 200 on-air television appearances, including being featured on Good Morning America, WIRED, and being an invited speaker and panelist for the White House COVID-19 Health Equity Summit.

With a passion for equity, innovation, and representation, Dr. Madison continues to inspire and influence through her entrepreneurial ventures, national leadership, and unwavering commitment to public health.


Cleanroom Environment Monitoring - Essentials for the Pharmacist
Louis S. Diorio, RPh, FAPhA

Bio:

Lou Diorio, RPh- Is a principal of LDT Health Solutions, Inc. (LDT), a medication safety & quality management consulting company with over 75 years of combined pharmacy expertise serving clients internationally.

Lou is a graduate of Long Island University’s Schwartz College of Pharmacy.  Lou is an adjunct Professor of Pharmacy Practice for the college and a preceptor of pharmacy students.  Lou is a member of the College’s Alumni Executive Board. Lou regularly lectures at Touro College of Pharmacy / Touro University.

Lou has extensive experience in IV and extemporaneous compounding, and lectures, writes, and consults on these topics; Lou lends his expertise to many State Boards of Pharmacy and is a nationally recognized subject matter expert on compounding, robotics & automation. Mr. Diorio has proctored the NYS Board of Pharmacy Licensure Exam (Part III) for Sterile Compounding. He has managed an FDA-registered cGMP manufacturing operation for Coram Healthcare (SoluNet™ LLC). 

Lou is an active APhA & ASHP member. Lou is a past Chair of APhA’s APPM Academy of Practice and Management, an APhA Fellow, and was the Chair of APhA’s Compounding Special Interest Group (SIG). He is also a past member of the Kappa Psi Pharmaceutical Fraternity Foundation Board of Directors, an advisor and past member of the New York State Council of Health System Pharmacists Research & Education Committee, and a member of the New Jersey Society of Health System Pharmacists Industry Relations Committee.

Since 1985 Lou has practiced in many clinical settings fulfilling many clinical & operational roles, including Chief Operating Officer of Hebrew Hospital Corporate Services, a multi-centered sub-acute and LTC hospital system in New York. He has also managed & practiced in homecare, hospital, and community settings.

During the Pandemic Mr. Diorio was asked by the New Jersey Board of Pharmacy to monitor the competency training, use of automation and quality management efforts at the four field hospitals serving the citizens of the State.

Lou is an Eagle Scout (with Silver Palm) and is still an active volunteer with the Boy Scouts of America.

Lou and his wife Kathi have two children Samantha (an attorney) and Michael (a civil engineer). 



Antibodies and Beyond: The Expanding Biologics Landscape
Holly R. Hamilton, PharmD, BCPS, AAHIVP

Bio:

Dr. Holly Hamilton is a board-certified clinical pharmacist at Yale New Haven Health, practicing in ambulatory digestive health with a focus on inflammatory bowel disease (IBD) and viral hepatitis. She earned her Doctor of Pharmacy degree from Sullivan University College of Pharmacy in 2013, followed by two years of post-graduate residency training: a PGY1 in Community Pharmacy with Sullivan University & Walgreens and a PGY2 in Pharmacotherapy with the University at Buffalo & Erie County Medical Center.

In her role at Yale, Dr. Hamilton provides comprehensive, patient-centered therapeutic management for individuals with complex GI conditions and contributes to interdisciplinary initiatives aimed at improving outcomes across the digestive health landscape.

She is also an adjunct professor and clinical preceptor, mentoring pharmacy students from multiple academic programs and supporting the next generation of clinical practitioners. She has presented at local, regional, and national conferences and collaborates with professional organizations to advance evidence-based practices in ambulatory care.

Outside of her clinical and academic work, Dr. Hamilton is a devoted mother, a published fantasy author, and an avid traveler who finds creative inspiration in new places and experiences.

Chantal Martin, MS, MBA

Bio:

Chantal Martin is the founder of the Three C Hub, where she offers counseling, coaching, and consulting to families and professionals navigating life transitions and leadership development. She also serves as Head of Technology Talent for a global fintech company, bringing more than 20 years of experience in human resources and executive leadership. Her work focuses on helping leaders, professionals, and families make clear decisions, execute against goals, and strengthen the leadership and professional skills required to perform at a high level.


The Art of Scientific Storytelling
Karen D. Corbin, PhD, RD

Bio:

Karen D. Corbin, PhD, RD, is an accomplished scientist and registered dietitian recognized for her leadership in obesity and metabolic research. She is also a sought after speaker and media contributor due to her impeccable scientific storytelling skills.

She earned her B.S. in Nutrition and Food Science from Florida State University and completed both her M.S. and Ph.D. in Molecular Medicine at the University of South Florida. Dr. Corbin currently serves as Associate Investigator and Obesity Program Lead  at AdventHealth’s Translational Research Institute, where she spearheads innovative clinical and translational studies.

Her career spans roles from dietetic practice to academia and research leadership, including positions at UNC Chapel Hill and AdventHealth. She has secured major NIH funding and has published extensively in high-impact journals, advancing knowledge on microbiome-driven energy metabolism, obesity and liver health.

Her contributions have earned numerous honors, including the ATHENA Orlando Emerging Leader Award (2024) and American Society for Nutrition’s Grand Prize for Postdoctoral Research. She has delivered keynote lectures at national and international conferences on obesity, gut microbiome, precision nutrition, and scientific storytelling. Dr. Corbin’s work exemplifies a commitment to translating and communicating science into impactful solutions for metabolic health.

Courtney C. Tackes, PharmD, MBA

Bio:

Courtney Tackes is the PGY1 Pharmacy Residency Program Director and a Supervising Pharmacist at St. Peter’s Hospital in Albany. She previously served as Assistant Dean for Experiential Education at Albany College of Pharmacy and Health Sciences, where she led experiential learning for over 1,000 placements annually. A past president of the Northeastern Chapter of NYSCHP, Courtney is passionate about advancing pharmacy education and mentoring student pharmacists and new practitioners.

Jennifer Cerulli, PharmD

Bio:
Jennifer Cerulli is Director of Experiential Education at Albany College of Pharmacy and Health Sciences (ACPHS), Albany NY. In this role, she is responsible for ensuring quality experiential learning opportunities for students and engaging in preceptor development. Her area of interest includes the application of Self-Determination Theory, Crucial Conversations and learning contracts within experiential learning to create autonomy supportive learning environments.

Dr. Cerulli completed her Bachelor of Pharmacy and Doctor of Pharmacy degrees from ACPHS, PGY1 Residency at Hospital of the University of Pennsylvania and Fellowship in Nutrition Support at ACPHS and AMC. Over her career, she has held various pharmacy patient care roles including serving as a community pharmacist, medication therapy management provider and specialty product clinical educator and specialty services pharmacist.
