The New York State Council of Health-system Pharmacists (NYSCHP) recognizes members and non-members for achievements and accomplishments in the pharmacy profession.

NYSCHP Awards:

Pharmacy Advocacy Award:

The recipient will receive complimentary registration to the NYSCHP Annual Assembly, accommodations for one night, and admission to the banquet.  The award will consist of a plaque.
Procedure and Guidelines
The Pharmacy Advocacy Award will be presented to a deserving member of the pharmacy community of New York State.  Any member can nominate a candidate for this award and members may also self nominate for the award.  Nominations must include a letter of support, a CV of the nominee and list of accomplishments highlighting the criteria of the award listed below. 
Criteria
The NYSCHP Pharmacy Advocacy award shall be given to an individual of high moral character and high professional ideals, who has made significant contributions to health-system pharmacy advocacy, an outstanding single achievement, or a combination of accomplishments benefiting health-system pharmacy advocacy.  They should include, but are not limited to the following examples:
  • Active participation in the NYSCHP Public Policy or Grassroots Advocacy Committees;
  • Implementing, leading, or expanding new programs such as local chapter committees, advocacy workshops, or other novel programs/events that increase member engagement;
  • Providing continuing education lectures to Council members on legislative and advocacy issues;
  • Publish or disseminate information on policy updates or legislative issues.
  • Sustained NY state pharmacy advocacy through legislative visits and/or involvement in Lobby Day; Candidates must be members of NYSCHP.
Awards Committee:  
The Chair will compile the entries and send copies to each Awards Committee member.  The Awards Committee will review all candidates and accompanying documents prior to selecting the most qualified candidate for the award. The final recommended awardee will be presented to the NYSCHP BOD for a vote at the next BOD meeting after the application deadline.
Presentation: 
The award will be presented at the Annual Assembly installation banquet.
The recipient may offer brief remarks upon acceptance of the award.
This award does not need to be given every year. 

Pharmacy Achievement Award:

The recipient will receive complimentary registration to the NYSCHP Annual Assembly, accommodations for one night, and admission to the banquet.  The award will consist of a lifetime membership as a Honorary Member in the NYSCHP and a bronzed plaque.
Procedure and Guidelines
The Board of Directors will present the Pharmacy Achievement Award to a deserving member of the pharmacy community of New York State.  The President will request input from each member of the Board of Directors with regard to potential candidates.  Each candidate must present his or her CV for review, including a list of accomplishments highlighting the criteria of the award listed below.  The Executive Committee will confer to determine candidates for the award.  The Board of Directors will vote on the candidate(s) to determine an award recipient.  
Criteria
The NYSCHP Pharmacy Achievement Award, intended to honor the accomplishments and contributions of a NYSCHP member, shall be given to an individual of high moral character and high professional ideals, who has made significant contributions to health-system pharmacy and through it, humanity and the public health.  They may include, but are not limited to accomplishments or achievements in:
  • Health-system pharmacy practice;
  • Health-system pharmacy education;
  • Health-system pharmacy administration;
  • Pharmaceutical research or development related to health-system pharmacy;
  • Scientific of professional pharmaceutical writing such as noteworthy published articles on pharmaceutical subjects;
  • Pharmaceutical journalism related to health-system pharmacy;
  • Public and/or inter-professional relations activities benefiting health-system pharmacy;
  • Pharmaceutical law or legislation, professional regulations, standards of professional conduct of pharmacy law enforcement;
  • Function and programs of NYSCHP.
Presentation: 
The award will be presented at the Annual Assembly installation banquet.

Corporate Achievement Award:

The award will consist of a plaque.
Procedure and Guidelines
The Corporate Achievement Award shall be given by NYSCHP to a company or corporate individual in recognition of accomplishments for, involvement in, and betterment of NYSCHP.  Nominations are submitted by the Director of Industry Relations on behalf of the Industry Relations Committee.
Criteria
The NYSCHP Corporate Achievement Award shall be given to an individual of high moral character and high professional standards who has been supportive of NYSCHP as evidenced any/all of the following:
  • Speaker sponsorship at the local/state level;
  • Active involvement within the Industry Relations Committee.
  • Participation in exhibit sessions at the Annual Assembly;
  • Participation in vendor networking session(s) at the Annual Assembly;
  • Participation in exhibit sessions at the Tristate Pharmacy Summit;
  • Participation in vendor networking session(s) at the Tristate Pharmacy Summit;
  • Other activities that better the organization.
Awards Committee:
The Chair will compile submissions and send copies to each Award Committee member with an accompanyingassessment sheet for evaluation.  The Awards Committee will forward completed assessment sheets to the Chair for review and compilation.  A summary will be presented to the NYSCHP BOD for a vote at the next BOD meeting after application deadline.
 
Presentation: 
The award will be presented at the Annual Assembly installation banquet.
This award does not need to be given every year.

Corporate Scholarship Essay Award:

The recipient will receive complimentary registration to the NYSCHP Annual Assembly, accommodations for one night, and admission to the banquet.
The recipient will receive recognition at the Annual Assembly Installation Banquet.
The recipient will receive a one year membership to NYSCHP.
The recipient’s essay will be published in the NYSCHP Newsbrief.
Procedures and Guidelines:
Essay question to be submitted by Director of Industry Affairs by mid-November to the NYSCHP office.
The essay question along with guidelines and requirements is forwarded to faculty liaisons.
Essays for consideration are then submitted to the school faculty liaisons who will forward to the NYSCHP office.
The Director of Industry Relations will compile the entries and submit them to Industry Relations Committee members along with an assessment sheet for evaluation. 
Criteria:
Recipient must be in final two (2) years of Pharmacy School in New York State. 
Student must compiled a 1000 word essay based on a topic chosen by the NYSCHP Industry Relations Committee.
Presentation:
The award will be presented at the Annual Assembly installation banquet.

New Practitioner Award:

The recipient will receive complimentary registration to the NYSCHP Annual Assembly, accommodations for one night, and admission to the banquet.  The award will consist of a plaque.
Procedure and Guidelines
The New Practitioner of the Year award will be presented to a deserving member of the pharmacy community of New York State.  The Presidents of the NYSCHP local chapters will be solicited each year to furnish names of candidates for this award.  The nomination will include a letter of support from the local chapter president, a CV of the nominee and a list of accomplishments.
Criteria
The NYSCHP New Practitioner of the Year Award recognizes an individual pharmacist for his or her leadership, vision, and continuous contribution to the practice of pharmacy in New York State in their first 1-5 years after formal education or residency training.  This award is meant to foster involvement from the beginning of their career.
Candidates must:
  • Be a practicing pharmacist in New York State.
  • Be a pledge level or active member of NYSCHP.
  • Be practicing 1-5 years from education or following pharmacist residency training programs (PGY1 or PGY2).
  • Demonstrate support of NYSCHP through active involvement either on the state or local level.
  • Demonstrate superior leadership, innovation, or clinical skills in the practice of pharmacy presentation.
Presentation: 
The award will be presented at the Annual Assembly installation banquet.
This award does not need to be given every year.
The NYSCHP BOD has the ability to add one time discretionary awards as needed to recognize the outstanding contributions of an individual or organization to the NYSCHP or healthcare.

Pharmacy Technician Award 

The recipient will receive complimentary registration to the NYSCHP Annual Assembly, accommodations for one night, and admission to the banquet.  The award will consist of a plaque.
Procedure and Guidelines:
The Pharmacy Technician award will be presented to a deserving member of the pharmacy community of New York State. Any NYSCHP member can nominate a candidate for this award and members may also self- nominate for the award. Each candidate must present his or her resume/CV for review, including a list of accomplishments highlighting how they meet the criteria of the award listed below. Each candidate also needs to submit a recommendation letter from a colleague or manager.
Criteria:
The NYSCHP Pharmacy Technician Award recognizes an individual technician for their exemplary leadership and contributions to the practice of pharmacy in New York State.
Candidates must:
  • Be an active member of NYSCHP
  • Be practicing in the State of New York for at least 2 years
    • Have active national certification (PTCB, NHA)
    • Have an active NYS registration
    • Demonstrate leadership in the practice of health system pharmacy
Contributions and accomplishments may include but are not limited to the following examples:
  • Implement, lead, or expand new programs such as local chapter committees, or other novel programs/events that increase member engagement
  • Demonstrate support of NYSCHP through active involvement either on the state and/or local level
    • Be actively involved in the NYSCHP Pharmacy Technician Committee as demonstrated by highlighted accomplishments
  • Speaker for a NYSCHP program
Presentation: 
The award will be presented at the Annual Assembly installation banquet.
This award does not need to be given every year.
