One Way: No Reentry! Navigating Transitions of Care and readmissions at a Community Hospital

  • Discuss the steps necessary to implement a pharmacy-driven TOC program

Description:

Effective care transitions are a vital and often overlooked component of patient care. As patients transition from a community setting to the hospital for acute care, and vice versa, they are particularly vulnerable to critical medication errors that may result in more complex and extensive care along with increased healthcare costs. Pharmacy-driven interventions during transitions of care have been shown to improve both patient health outcomes and financial outcomes for hospitals. In recent years, there has been increased interest in developing TOC programs. This presentation will discuss the benefits of and steps to implement a pharmacy-driven TOC program at a community hospital.
